Abstract:Umbilical cord blood(UCB)autotransfusion is a perioperative blood conservation strategy with considerable clinical potential, particularly for neonates with congenital heart disease(CHD)who require open-heart surgery. This technique utilizes cord blood collected at birth as a natural autologous blood source for priming the cardiopulmonary bypass(CPB)circuit and for intraoperative transfusion, effectively reducing or even eliminating the need for allogeneic blood products. The standardized implementation of this approach has positive implications for reducing transfusion-related complications and improving perioperative safety and quality of life in children with CHD.Drawing on current domestic and international literature and taking into account China's healthcare context and clinical practices, this article systematically elaborates on the application of UCB autotransfusion within an integrated peripartum management model for CHD.It outlines the indications and contraindications, emphasizing the establishment of a comprehensive management protocol covering UCB collection, storage, quality control, transfusion, and adverse reaction management.
