Research progress on endoplasmic reticulum autophagy in heart failure

Abstract:Heart failure is a complex clinical syndrome caused by various etiologies that lead to structural and/or functional abnormalities of the heart,resulting in impaired ventricular systolic and/or diastolic function.The endoplasmic reticulum,a critical organelle in eukaryotic cells responsible for protein folding,processing and transport,plays a vital role in maintaining cellular homeostasis.Endoplasmic reticulum autophagy(ERA),a selective form of autophagy,regulates endoplasmic reticulum membrane content by eliminating excess or dysfunctional endoplasmic reticulum structures and associated macromolecular complexes.ERA is widely involved in numerous physiological processes,including metabolic regulation,immune responses,inflammatory signaling and cell survival,and has emerged as a focal point in autophagy research.Simultaneously,ERA serves as a key endogenous mechanism in alleviating endoplasmic reticulum stress(ERS),and the two processes work synergistically to maintain endoplasmic reticulum homeostasis.Recent studies have revealed that both ERA and ERS exert protective effects on cardiomyocytes in heart failure but the excessive stress may exacerbate myocardial injury.Therefore,the interplay between ERA and ERS plays a crucial role in the pathogenesis of heart failure.This review first outlines the cell biological characteristics and core receptors of ERA,then summarizes its regulatory mechanisms in heart failure,aiming to provide a theoretical foundation for further mechanistic studies and potential therapeutic targets in heart failure.
