Abnormal expression of serum biomarkers in assessing cardiac function and prognosis in heart failure patients complicated with pulmonary infection

Abstract:Objective To investigate the abnormal expression of serum matrix metalloproteinases(MMPs),N-terminal pro-B-type natriuretic peptide(NT-proBNP)and interleukin-6(IL-6)in inducing cardiac impairment and its impact on prognosis in patients with heart failure complicated by pulmonary infection.Methods A total of 154 patients with chronic heart failure(CHF)admitted to Xingtai Central Hospital were selected and divided into the infection group(n=108)and non-infection group(n=46)based on the presence or absence of pulmonary infection.Serum levels of MMPs,NT-proBNP,IL-6 and cardiac function were measured in both groups.The infection group was further divided into the good prognosis group and the poor prognosis group based on the occurrence of adverse events during hospitalization and within 6 months of telephone follow-up.The impact of MMPs,NT-proBNP and IL-6 on the prognosis of CHF patients with pulmonary infection was analyzed,and the receiver operating characteristic(ROC)curve was used to evaluate their predictive value.Results The levels of MMP-2,MMP-9,NT-proBNP and IL-6 in the infection group were significantly higher than those in the non-infection group[(250.32±30.32)ng/ml vs.(196.86±20.34)ng/ml,(422.23±38.67)ng/ml vs.(326.78±31.43)ng/ml,(10.32±2.01)pg/ml vs.(7.32±1.82)pg/ml,(18.02±3.12)ng/L vs.(11.32±2.13)ng/L,P<0.05].The heart rate(HR),left ventricular end-diastolic diameter(LVEDD)and left ventricular end-systolic diameter(LVESD)in the infection group were significantly higher than those in the non-infection group[(86.23±7.32)times/min vs.(78.23±6.98)times/min,(64.32±6.72)mm vs.(58.32±6.02)mm,(52.56±5.21)mm vs.(46.01±4.89)mm,P<0.05],while left ventricular ejection fraction(LVEF)was significantly lower than that in the non-infection group(42.67±3.21 vs.52.63±4.02,P<0.05).In the infection group,66 patients had a good prognosis,while 42 had a poor prognosis.Univariate analysis showed that the levels of MMP-2,MMP-9,NT-proBNP and IL-6 in the good prognosis group were significantly lower than those in the poor prognosis group[(236.42±27.86)ng/ml vs.(258.32±31.02)ng/ml,(402.32±34.67)ng/ml vs.(429.67±37.89)ng/ml,(8.94±1.96)pg/ml vs.(10.96±2.21)pg/ml,(16.24±2.86)ng/Lvs.(18.89±3.54)ng/L,P<0.05].Multivariate Logistic regression analysis indicated that MMP-2,MMP-9,NT-proBNP and IL-6 were the risk factors affecting the prognosis of CHF patients with pulmonary infection.ROC curve analysis further validated that the areas under the curve(AUC)for MMP-2,MMP-9,NT-proBNP,IL-6 and their combination in predicting the prognosis of CHF patients with pulmonary infection were 0.617,0.736,0.834,0.728 and 0.930,respectively,with the combined prediction outperforming individual indicators.Conclusion Abnormally elevated serum levels of MMPs,NT-proBNP and IL-6 are valuable for assessing cardiac impairment and prognosis in CHF patients with pulmonary infection,and they may serve as potential biomarkers for clinical treatment and prognosis management.
