Assessment of microcirculation function in patients with acute ST-segment elevation myocardial infarction after percutaneous coronary intervention

Abstract:Percutaneous coronary intervention(PCI)for early reperfusion of the epicardial coronary arteries is the guideline recommended treatment for ST elevation myocardial infarction(STEMI).More than 95%of PCI procedures can successfully restore epicardial coronary artery flow.Although angiography showed complete patency of the epicardial coronary artery,the distal microvascular perfusion of the coronary artery was not fully restored in about half of the patients.Previous studies have demonstrated that coronary microcirculation dysfunction(CMD)is associated with adverse prognostic events in patients.Therefore,selecting an appropriate assessment method for CMD will be of great significance for the prognosis and early assessment of STEMI patients.At present,there are many methods for assessing coronary microcirculation dysfunction,but due to the accuracy or accessibility of clinical application,there is a lack of a unified conclusion in clinical practice.This review will systematically retrospect previous studies on CMD,and introduce the evaluation indicators of CMD function based on coronary angiography from both invasive and non-invasive aspects.At the same time,the principle,accuracy,prognostic study,advantages and limitations will be analyzed to provide reference for clinical evaluation of coronary microcirculation function and reasonable stratification of STEMI patients after PCI.
