Lipid management in very-high-risk atherosclerotic cardiovascular disease patients

Abstract:Atherosclerotic cardiovascular disease(ASCVD)patients at very high risk represent a primary contributor to morbidity and mortality in China, where dyslipidemia management serves as a critical modifiable factor in prevention strategies. However, significant challenges persist in optimizing lipid control for this population, particularly due to complexity in defining diagnostic criteria and poorly characterized disease spectrum, which substantially impede effective screening and diagnosis.Furthermore, conventional static risk assessment systems fail to capture dynamic residual risk trajectories, while intervention strategies lack robust evidence derived from Chinese populations. This commentary examines these core challenges and proposes optimized approaches, emphasizing that the establishment of ethnicity-specific diagnostic stratification, dynamic risk evaluation protocols, intensive goal-oriented therapy, and comprehensive lifelong management constitute critical pathways toward achieving optimal lipid control and improved cardiovascular outcomes in this vulnerable cohort.
