Hemodynamic study of ascending aorta in patients with bicuspid aortic valve based using four-dimensional magnetic resonance imaging

Abstract:Objective To investigate the hemodynamics of the ascending aorta in patients with bicuspid aortic valve(BAV)using four-dimensional magnetic resonance imaging(4D Flow MRI)and the relationship between abnormal blood flow patterns and aortic dilatation.Methods From August 2023 to October 2024,a prospective study was conducted involving 25 BAV patients at Beijing Anzhen Hospital,Capital Medical University.Aortic blood flow data were acquired using a 4D Flow MRI sequence,and the hemodynamic parameters of the ascending aorta were analyzed using CVI 42 post-processing software.Results The study included 25 patients(19 males)with an average age of(55.4±10.9)years.The flow analysis revealed significant eccentric jets and vortices in the ascending aorta of BAV patients,with notable increases in circumferential and axial wall shear stress(WSS)at the widest part of the aorta.In cases of concurrent ascending aortic aneurysm,WSS at this location significantly decreased.When accompanied by aortic valve stenosis,all the hemodynamic parameters increased.No significant differences were observed in blood flow distribution or hemodynamic parameters between two-sinus BAV and left-right fusion BAV.Conclusions BAV patients exhibit marked abnormal blood flow in the ascending aorta,which is exacerbated by aortic valve stenosis.WSS is significantly reduced in patients with aortic dilation,and circumferential WSS may serve as a predictor of aortic dilatation.
