Value of cardiac nuclear magnetic resonance parameters and serum Mir-21 in early recurrence of atrial fibrillation after radiofrequency ablation

Abstract：Objective To investigate the predictive value of cardiac magnetic resonance(CMR)imaging parameters and blood microRNA-21(miR-21)in early recurrence of atrial fibrillation(AF)after radiofrequency ablation(RFA).Methods A total of 125 AF patients who received electrophysiological examination and RFA treatment in Chengdu Third People's Hospital from January 2021 to January 2022 were retrospectively selected as the research objects.All the patients were followed up for 3 months and divided into the recurrence group(n=41)and non-recurrence group(n=84)according to whether they had early recurrence.The clinical data and CMR parameters were collected and the receiver operating characteristic(ROC)curve was used to analyze the predictive value of CMR parameters and Mir-21 for early recurrence after RFA in AF patients.Results The left atrial volume at maximum(LAVmax)in the recurrence group was higher than that in the non-recurrence group[(80.64±28.01)ml/min vs.(58.16±21.41)ml/min].The absolute values of left atrial ejection fraction(LAAEF),left atrial total strain(Es),peak systolic strain rate(SRs),passive strain(Ee),and early diastolic negative strain rate(SRe)are all lower than those in the non-recurrence group[(12.53±4.43)％vs.(17.57±3.64)％、(9.48±8.04)％vs.(15.12±9.95)％、(0.62±0.40)％/s vs.(0.88±0.49)％/s、(6.65±4.06)％vs.(9.41±5.34)％、(-0.51±0.52)％/s vs.(-0.84±0.61)％/s,P＜0.05].After incorporating the indicators with significant differences in univariate analysis into the Logistic regression model,elevated levels of miR-21 and CMR parameters LAV max and LAAEF were identified as the risk factors for early recurrence of atrial fibrillation(AF)after RFA(OR=0.121,0.868,0.413,P＜0.05).When these three indicators were included in the receiver operating characteristic(ROC)model,the combined measurement demonstrated the highest predictive value,with an area under the curve(AUC)of 0.879,a sensitivity of 87.80％and a specificity of 75.00％for diagnosis.Conclusions The left atrial function of AF patients is weakened and myocardial cells are damaged.The use of CMR parameters combined with miR-21 can predict early recurrence after RFA to a certain extent.
