Predictors of stroke after radiofrequency catheter ablation of atrial fibrillation

Abstract:Objective To seek the risk factors for stroke after catheter radiofrequency ablation in patients with nonvalvular atrial fibrillation(NVAF).Methods A total of 500 patients with NVAF who underwent percutaneous catheter radiofrequency ablation in Fuwai Huazhong Cardiovascular Hospital and the Second People's Hospital of Pingdingshan City,Henan Province from January 2018 to December 2019 were continuously enrolled.The medical history data,type of atrial fibrillation,transthoracic echocardiographic parameters,CHA2DS2-VASc and left atrial appendage morphology were recorded.All the patients underwent regular follow-up,and the stroke events were inquired and recorded in detail.Results During(24.8±5.9)months' follow up,there were 47 cases of ischemic stroke(9.4%)occurred with no hemorrhagic event.Cox regression analysis showed that persistent atrial fibrillation was the independent risk factor related with ischemic stroke(HR=2.674,95%CI 1.323-5.404,P=0.006),whereas chicken-wing left atrial appendage(LAA)morphology was(HR=0.183,95%CI 0.067-0.500,P=0.001)was an independent protective factor.The CHA2DS2-VASc was not significantly associated with the occurrence of postoperative stroke.Conclusion For NVAF patients undergoing surgical treatment,those with persistent atrial fibrillation have a higher risk of postoperative ischemic stroke,while those with a chicken wing shaped LAA have a lower risk of postoperative ischemic stroke.
