Clinical application progress of cardiac contractility modulation in treatment of chronic heart failure

Abstract：Cardiac contractility modulation(CCM),as a novel therapeutic approach,addresses the limitations of device-based treatment for heart failure,particularly for patients who do not respond adequately to medication and do not meet the criteria for cardiac resynchronization therapy(CRT)implantation.CCM delivers electric signals during the absolute refractory period of the heart,enhancing intracellular calcium levels to increase myocardial contractility while improving ventricular remodeling without increasing myocardial oxygen consumption.Current clinical studies have demonstrated that CCM significantly improves cardiac function,enhances quality of life and reduces hospitalization rates in heart failure patients.Additionally,it shows a favorable safety profile,with few severe adverse events reported during the treatment.This paper comprehensively reviews the mechanism,clinical efficacy and safety of CCM.
