Prognostic value of H2FPEF score in patients with non-ejection fraction reduced heart failure

Abstract:Objective To investigate the predictive value of the H2FPEF score in patients with non-ejection fraction reduced heart failure(non-HFrEF).Methods A to of 200 patients with heart failure with preserved ejection fraction treated at The Emergency General Hospital between February 2021 and February 2022 were enrolled and categorized into three groups based on their H2FPEF scores:the low group(0-1),the intermediate group(2-5)and the high(6-9).The participants were followed up for a period of two years.The primary endpoint was all-cause mortality,while the secondary endpoints comprised cardiac death,readmission rates and ICU admissions.Results There were 51 patients in the low group,102 patients in the middle group,and 47 patients in the high group.There were significant differences in all-cause mortality,cardiac death,ICU admission rate and heart failure re-hospitalization rate among the three groups(P<0.01).Multivariate Cox regression analysis showed that hypertension,atrial fibrillation and H2FPEF score(HR=2.251,95%CI 1.672-3.031,P<0.001)were the independent predictors of all-cause death.The analysis of subjects'working characteristic curves showed that H2FPEF score had better ability to predict all-cause death and heart failure re-hospitalization,and the area under the curve was 0.890(P=0.001)and 0.837(P=0.001),respectively.The optimal threshold value of H2FPEF score to predict all-cause death within 2 years was 4.5 points.The optimal threshold for predicting heart failure re-hospitalization events was 2.5 points.Conclusions The H2FPEF score is an effective tool for predicting the prognosis of the patients with heart failure with reduced non-ejection fraction.
