Correlation between platelet count/lymphocyte and severity of coronary artery disease and short-term prognosis in patients with ST segment elevation myocardial infarction

Abstract:Objective To investigate the correlation between platelet count/lymphocyte ratio(PLR)and the severity of coronary artery lesions in patients with ST elevation myocardial infarction and the predictive value of PLR for two months mortality. Methods 120 cases of patients with ST elevation myocardial infarction in our hos-pital from April 2013 to April 2015 undergoing emergency percutaneous coronary artery interventional therapy were enrolled and according to the SYNTASX score evaluation,120 patients were divided into severe coronary artery le-sion group and non severe coronary artery lesion group,severe coronary artery lesion group(SYNTASX integral>25)72 cases,non severe coronary artery lesion group(SYNTASX score less than or equal to 25)in 48 cases. Ac-cording to the three points of PLR,120 patients were divided into high PLR group(PLR=210)42 cases in group PLR(210>PLR>90)21 cases,low PLR group(PLR=90)in 57 cases. The correlation between the change of PLR level and the degree of coronary artery lesions and the predictive value of PLR for two month all-cause mortality were analyzed. Results The platelet count in patients with non severe coronary artery disease[(189.6±51.2)× 109/L vs.(214.3±84.1)×109/L]and PLR(95.4 vs 241.1)were significantly lower than those in severe coronary artery disease group(P<0.05). Lymphocyte count[(2.1±0.3)×109/L vs.(1.8±1.2)×109/L]and high density lipoprotein cholesterol[(1.2±0.5)mmol/L vs.(0.9±0.6)mmol/L]in non severe coronary artery disease group were significantly higher than those in severe coronary artery disease group(P<0.05). Non severe coronary artery lesions group and multi vessel disease proportion(27.1% vs. 37.5%),Tirofiban usage(31.2% vs. 59.7%),the use rate of thrombus aspiration(20.8% vs. 36.1%),2 months due to cumulative mortality(4.2% vs. 9.7%)were significantly lower than that of severe coronary artery disease group,the difference was statistically significant(P<0.05). The risk ratio of SYNTAX was 1.4,and the 95% confidence interval was(1.2-1.9),and it was associated with the death of patients in 2 months(P<0.05). The risk ratio of PLR was 1.2,and the 95% confidence interval was(1.3-2.0),and it was associated with the death of patients in two months(P<0.05). Conclusion PLR is closely relat-ed to the severity of coronary artery lesions in patients with ST elevation myocardial infarction,and has predictive value for the prognosis of patients.
