Effect of systolic blood pressure fluctuation on prognosis in patients with atrial fibrillation associated with hypertension
Abstract:Objective To investigate the significance of pressure fluctuation on prognosis in patients with atrial fibrillation associated with hypertension.Methods Patients with atrial fibrillation and hypertension were selected during 2012-2014 admitted in Xinjiang Uygur Autonomous Region people's Hospital,and the change of systolic blood pressure,means of systolic blood pressure,the occurrence of sudden cardiac death,acute coronary syndrome,chronic heart failure,cerebral embolism and peripheral vascular embolism were followed up.Results 334 cases were finished follow-up,including 71.3% of men,43.7% of paroxysmal atrial fibrillation,median follow-up of 1.5 years,and a median fluctuation of blood pressure was of 10.8 mm Hg.In the follow-up process,with the increase of systolic blood pressure fluctuations,the end point event rate was increasing(Primary endpoint:HR=1.54,95%CI 2.10-3.12,P=0.01;Secondary endpoint:HR=1.43,95%CI 1.87-3.02,P=0.01).In the highest quartile of systolic pressure fluctuation,the primary end point and secondary end point event rate was significantly higher than the systolic blood pressure fluctuations in the lowest quartile (Primary endpoint:HR =1.82,95%CI 1.327-4.812,P<0.01,Secondary endpoint:HR=l.74,95%CI 1.487-4.312,P<0.01),and the patients were followed up more times predictive value of systolic blood pressure fluctuations.The prognosis of patients with atrial fibrillation was greater,the predictive value of pressure was higher in patients with more obvious average shrinkage (P<0.05).Conclusion Systolic pressure fluctuations can affect the prognosis of patients with atrial fibrillation associated with hypertension,and the greater the fluctuation of systolic pressure,the worse the prognosis of patients with atrial fibrillation.
