Value of serum erythropoietin level on predicting infarct size in patients with acute myocardial infarction
Abstract:Objective We investigated whether a higher serum erythropoietin(EPO) level in patients with acute ST segment elevation myocardial infarction (STEMI) subjected to successful primary percutaneous coronary intervention (PCI) can predict a smaller infarct size determined by creatine kinase (CK) release.Methods 183 patients with STEMI who received successful primary PCI within 12 h from the onset of AMI were prospectively studied.Blood samples were collected to examine the serum EPO and CK level.Results Levels of peak CK,cumulative CK release and pre-infarction angina in below-median EPO group were (3.63±1.87)×103U/L,(107.5± 82.3) × 103 U· h-1· L-1 and 22.9% (19/83) respectively,in above-median EPO group were (2.15± 1.23) × 103U/L,(68.1±50.4)×103 U·h-1·L-1,40.0%(32/80) respectively,the differences were significantly (P<0.01,P<0.01,P=-0.019).The level of EPO was correlated negative with CK peak (r=-0.503,P<0.05) and cumulative CK release (r=-0.441,P<0.05).The binary regression analysis shown,EPO (OR=0.750,95%CI 0.606-0.928,P=0.008) enter the coronary collateral grade regression equation eventually.A stepwise multiple regression analysis revealed that the serum EPO level (3=-0.504,P<0.05) as well as pre-infarction angina(β=-0.225,P<0.05) was an independent predictor for the cumulative CK release.Condusion These data suggest that a high endogenous EPO level can predict a smaller infarct size in patients with STEMI subjected to successful primary PCI.This might be attributed to the potentially protective effect of endogenous EPO against ischemia-reperfusion injury in humans.
