The coronary artery lesion characteristics and clinical features of acute myocardial infarction complicated the history of cerebral infarction

Abstract：Objective To investigate the coronary artery lesion characteristics and clinical features of acute myocardial infarction complicated the history of cerebral infarction.Methods The study included 99 AMI cases.According to whether complicated the history of cerebral infarction is or not,these cases were divided into two groups,including 13 cases of cerebral infarction group and 86 cases of no-cerebral infarction group.The infarction area and the characteristics of the infarction related artery of AMI cases complicated the history of cerebral infarction were observed.In all the cases and controls,CK-MB concentrations were measured continuously.The area under the curve of CK-MB concentration,incidence of malignant arrhythmias and mortality during hospitalization were studied and compared between two groups.Results In this group of patients of AMI complicated the history of cerebral infarction accounted for 13.1％.There were 7 cases of anterior myocardial infarction in the cerebral infarction group which criminals were left anterior descending arteries.There were 6 cases of right ventricular and inferior wall myocardial infarction which criminals were right coronary arteries.The cerebral infarction group has no obvious specificity of coronary lesion vessels,but the criminal vascular area was almost located proximally.It indicates that the condition was relatively heavy.The incidence of malignant arrhythmia was 23.08％ in the cerebral infarction group and 8.14％ in the no-cerebral infarction group.The mortality rate during hospilization was 23.08％ in the cerebral infarction group and 6.98％ in the no-cerebral infarction group.The CK-MB area under the curve,the incidence of malignant arrhythmia and the hospital mortality in the cerebral infarction group analysis demonstrated higher than the no-cerebral infarction group.Conclusion The criminal vascular in the cerebral infarction group are mostly located proximally.And the infarction area is relatively large and heavy.It demonstrated that these cases are prone to malignant arrhythmia.As a whole,the prognosis is poor in the cerebral infarction group.
