The in-hospital outcomes of stent post-dilation in primary PCI for patients with ST-segment elevation myocardial infarction and multivessel disease
Abstract:Objective To investigate the safety and efficacy of postoperative PCI dilation in ST-segment elevation myocardial infarction (STEMI) patients with multivessel disease.Methods A total of 1081 STEMI patients with multivessel disease that treated in Beijing Anzhen Hospital from 2005 to 2014 were continuous enrolled,455 of which had performed post-dilation.According to dilation or not,patients were divided into two groups.The primary adverse cardiac and cerebrovascular events were compared between the two groups.Results There were no statistically significant differences in demographic characteristics,myocardial infarction sites,coronary artery lesion characteristics (infarct-related vessels,non-infarct-related blood vessels,coronary artery stenosis ≥ 70% lesion),perioperative medication,cardiac function grade between two groups,so were the mean stent diameter,the residual stenosis ≥ 30% and the bifurcation stent.The proportion of thrombosis in post-dilation group was lower than that of the control group.In post-dilation group,the time of onset to pPCI and the total length of the stents were longer.There was larger number of stents or overlapping stents in post-dilation group.There was no statistically significant difference between the two groups in the incidence of no-reflow,acute/subacute thrombus and IABP in the stent.Also,in-hospital events such as cardiopulmonary death,acute myocardial infarction,posterior angina pcctoris,infarct-related revascularization and stroke show no significant statistically difference.The average length of stay in the post-dilation group was shorter than that in control group.Conclusion Since post-dilation did not increase immediate or in-hospital adverse events.It is safe and feasible for STEMI patients with multivessel disease to perform post-dilation in primary PCI.
