The evaluation for the degree of coronary artery disease by using two-dimension speckle tracking imaging
Abstract:Objective To measure global longitudinal strain (GLS), global radial strain (GRS), global circumferential strain(GCS) of the left ventricle and to evaluate the degree of coronary artery disease by using two-dimension speckle tracking imaging (2D-STI), to explore the noninvasive method of 2D-STI in evaluating the severity of coronary artery disease. Methods 60 patients who were in hospital for chest pain and primary diag-nosed as CAD were chosen as experimental group. All patients had at least one main coronary artery whose degree of lesion≥50%. 30 health people without organic heart disease were chosen as control group. Results With the increasing of the degree of coronary lesion in experimental groups, the absolute value of GLS, GRS and GCS grad-ually decreased(P<0.05). The absolute value of GLS, GRS and GCS and the Gensini score had a significant nega-tive correlation(Pearson correlation coefficient were r=-0.823,P<0.01,r=-0.775,P<0.01,r=-0.763,P<0.01). The cut-off point of GLS to diagnose the severe coronary lesion was -13.5%, and the sensitivity was 95.5% while specificity was 73.7%. The Youden Index was the highest one for 0.692. GRS and GCS had lower Youden Index and sensitivity but higher specificity. Conclusion With the increasing of the degree of coronary lesion, the abso-lute value of GLS, GRS and GCS gradually decreased. The global strain of left ventricular measured by 2D-STI has good correlation with coronary angiography Gensini Score, which can be used for noninvasive assessment of coronary artery lesion. 2D-STI provides a basis for noninvasive quantitative assessment of coronary artery lesion in coronary heart disease.
