Influence of obstructive sleep apnea hypopnea syndrome on heart rate variability in very elderly hypertensive patients

Abstract:Objective To analysis the influence of obstructive sleep apnea hypopnea syndrome(OSAHS) on heart rate variability in very elderly hypertensive patients. Methods A total of 120 very elderly hypertensive patients suspected OSAHS in our department from January 2013 to July 2015 (male 71 cases,female 49 cases) were collected. All of them checked polysomnography and 24-hour Holter monitoring. According apnea-hypopnea index(AHI),they were divided into four groups: Non-OSAHS group(control group,28 cases)and mild OSAHS group(mild group,36 cases),moderate OSAHS group(moderate group,30 cases)and severe OSAHS group(se-vere group,26 cases). The differences of their clinical features,biochemical index,sleep monitoring index and heart rate variability were compared. The influence of OSAHS on heart rate variability in very elderly hypertensive patients was analyzed. Results ⑴The daytime standard deviation of NN intervals,the nighttime standard devia-tion of NN intervals,the daytime square root of the mean squared difference of successive NNs and the nighttime square root of the mean squared difference of successive NNs of the four groups were significantly reduced with the aggravation of sleep-disordered breathing. The differences were statistically significant (P<0.05). ⑵The daytime low frequency and the nighttime low frequency of mild,moderate,severe group were higher than control group, and the differences were statistically significant (P<0.05). The daytime low frequency and the nighttime low fre-quency of moderate、severe group were higher than mild group,and the differences were statistically significant(P<0.05). But the differences of the daytime low frequency and the nighttime low frequency between moderate group and severe group were not statistically significant (P>0.05). The daytime high frequency,the nighttime high fre-quency of the four groups were significantly reduced with the aggravation of sleep-disordered breathing,and the differences were statistically significant(P<0.05). The daytime LF/HF and the nighttime LF/HF of the four groups were significantly rised with the aggravation of sleep-disordered breathing,and the differences were statistically significant(P<0.05). Conclusion Maybe OSASH is an important factor to reduce heart rate variability of the very elderly hypertensive patients.
