Application and comparison in predicting acute kidney injury after off-pump coronary bypass grafting using three kinds of risk scores
Abstract:Objective To investigate the application and comparison in predicting acute kidney injury (AKI)after off-pump coronary bypass grafting(OPCAB)using three kinds of risk scores used commonly in clinical medicine. Methods According to the international AKI KDIGO diagnosis criteria,340 patients who were enrolled and received OPCAB from May 2013 to December 2015 are categorized into post-operation AKI group(n=65)and non AKI group (n=275). Comparisons are made with STS, ES, ESⅡ risk scores, clinical characteristics, complications and mortality. Results ESⅡ risk score in AKI group was significantly higher [(5.37 ±4.37)% vs (3.17±3.09)%,P=0.0003]than that in non-AKI group,and there was no significant statistic differences in STS and ES scores between two groups. Significant differences are found in body mass index (BMI)(P=0.014),pre-operation left ventricular ejection fraction(LVEF)(P=0.007),pre-operation serum creatinine(P=0.001)and intra-aortic balloon pump(IABP)(P<0.01)between groups. Post-operation myocardial infarction rate(7.69% vs 0.36%, P<0.01),infection rate (46.15% vs 10.91%,P<0.01)and mortality rate are significantly higher in AKI group. Conclusion ESⅡ score can be used in predicting cardiac surgery post-operative AKI. ESⅡ score is superior to STS and ES scores in predicting post-operative AKI.
