The association between pulse pressure and in-hospital prognosis in patients with acute aortic dissection

Abstract:Objective To study the relationship between pulse pressure and in-hospital prognosis in patients with acute aortic dissection.Methods Patients with acute aortic dissection were selected from The First Affiliated Hospital of Anhui Medical University from January 2013 to December 2015,totally 270 cases.The pulse pressure on admission was divided into four groups:group A (lower p-ulse pressure,PP ≤ 39 mm Hg),group B (normal pulse pressure,PP 40-56 mm Hg),group C (mildly elevated pulse pressure,PP 57-75 mm Hg) and group D (markedly elevated pulse pressure,PP ≥76 mm Hg).The clinical data of four groups and further study factors related with in-hospital prognosis were analyzed by logistic regression analysis.Results Compared with the other three groups,the differences in average age,heart rate,Stanford A of group A have statistical significance (P<0.05),differences in syncope/coma,low blood pres-sure (SBP< 100 mm Hg),eliac trunk ischemia,pericardial effusion,dissection rupture and in-hospital mortality of group A have statistical significance (P<0.05).Further analysis by logistic regression for inhospital prognosis risk factors werethe renal dysfunction,Stanford type A,pericardial effusion,lower pulse pressure (PP ≤ 39 mm Hg),and surgery was the protective factor for in-hospital prognosis.Conclusion Lower pulse pressure(≤ 39 mm Hg) is associated with in-hospital prognosis of acute aortic dissection,and can predict in-hospital prognosis.
