Research advances on epigenetic mechanisms of temozolomide resistance in glioblastoma and emerging therapeutic strategies

Abstract:Glioblastoma(GBM)is one of the most aggressive malignant brain tumors.Although temozolomide(TMZ)is the most widely used chemotherapeutic agent,treatment failure often occurs due to drug resistance.This review summarizes the role and mechanisms of epigenetic regulation in TMZ resistance,including DNA methylation,histone modifications(mainly including acetylation,methylation,and lactylation)that promote resistance by regulating MGMT expression,DNA damage repair,mismatch repair function,and cancer stem cell properties;and non-coding RNAs(including long non-coding RNAs,microRNAs,and circular RNAs)that mediate resistance through various mechanisms such as influencing MGMT transport,DNA repair,signaling pathways,and cellular metabolism.Current targeted epigenetic therapeutic strategies show promise in reversing TMZ resistance.Future research should focus on elucidating the collaborative epigenetic regulatory networks and advance individualized precision treatment strategies to improve the prognosis of GBM patients.
