Localization technique for subcortical gliomas based on recognition of brain surface topological features

Abstract:Objective To explore a precise localization technique based on the recognition of brain surface topological features,aiming to enhance the extent of tumor resection and reduce postoperative dysfunction.Methods Three-dimensional reconstruction was performed using 1 mm-thick MRI data to construct projection models of the lesion on the brain surface and the lateral fissure.Topological features such as sulci,gyri,and blood vessels were identified to plan individualized cerebral cortical incisions.DTI neural tract imaging was integrated to simultaneously display the spatial relationship between the tumor and critical functional tracts.Results Preoperative localization and intraoperative anatomy were highly consistent in three cases of gliomas located in different regions(frontal lobe,temporal lobe,and insular subcortex).The length of cortical incisions was shortened,the protection of vascular and neural structures was remarkable,tumor resection was satisfactory,and no severe postoperative complications occurred.Conclusions The topological localization technique provides a more intuitive intraoperative lesion localization method through a brain surface topological feature matching mechanism,overcoming brain shift errors associated with traditional neuronavigation.It offers a novel strategy for high-precision glioma surgery,achieving a balance between"anatomical resection"and"functional preservation".
