Relationship of peripheral blood cell counts and related inflammatory markers with glioma grading

Abstract:Objective Based on the WHO Classification of Tumors of the Central Nervous System(5th Edition),this study aimed to explores the predictive value of peripheral blood cell counts and related inflammatory markers for glioma grading,providing a new evidence for preoperative glioma staging.Methods Clinical data of 302 glioma patients were analyzed retrospectively.Clinical information,including complete blood cell counts and serum biochemical test results,was collected to calculate related inflammatory markers.Immunohistochemistry and molecular testing results were also collected.Integrated diagnosis of gliomas was performed according to the WHO Classification of Tumors of the Central Nervous System(5th Edition),and the patients were divided into high-grade glioma(HGG)and low-grade glioma(LGG)groups.Univariate and multivariate logistic regression analyses were used to identify independent predictors of glioma grade.A predictive model was established,and its performance was evaluated using receiver operating characteristic(ROC)curve analysis.Results Univariate analysis revealed that age,absolute neutrophil count,absolute monocyte count,neutrophil-to-lymphocyte ratio(NLR),monocyte-to-lymphocyte ratio(MLR),systemic immune-inflammation index(SII),and systemic inflammation response index(SIRI)were significantly higher in HGG patients than in LGG patients.Multivariate analysis identified age,absolute neutrophil count,NLR,SII,and SIRI as independent predictors of glioma grade(all P<0.05).Among the predictive models,the combination of age and absolute neutrophil count demonstrated the highest predictive efficacy(AUC=0.777).Conclusions Under the WHO Classification of Tumors of the Central Nervous System(5th Edition)grading criteria,preoperative absolute neutrophil count and related inflammatory markers are significantly associated with glioma grade.HGG patients exhibit a stronger systemic inflammatory state.Absolute neutrophil count and related inflammatory markers can serve as auxiliary tools for preoperative evaluation of glioma grading in the new classification system.
