Research progress of high-frequency oscillations as biomarkers of epileptogenic zones:from mechanism to clinical application

Abstract:Precise localization of the epileptogenic zone is crucial for successful epilepsy surgery and improved patient outcomes.In recent years,high-frequency oscillations(HFOs)have garnered significant attention as potential biomarkers for the epileptogenic zone,with their underlying pathophysiological mechanisms being increasingly elucidated.Studies indicate that the generation of HFOs is closely associated with the synchronous firing of pathological neuronal clusters.This process involves mechanisms such as neural circuit reorganization and excitatory/inhibitory imbalance,thereby conferring high spatial and pathological specificity.Currently,the detection of HFOs primarily relies on intracranial electroencephalography(iEEG),especially stereo-electroencephalography(SEEG),which has demonstrated considerable advantages in localizing the epileptogenic zone.Regarding analytical methods,the introduction of novel techniques like automated detection and machine learning has further enhanced the efficacy and reliability of HFO identification.Although HFOs hold significant potential in the presurgical evaluation for epilepsy,their clinical application still faces numerous challenges,including the differentiation between physiological and pathological HFOs and the limitations of non-invasive detection techniques.This review systematically summarizes the current understanding of the generation mechanisms,detection methodologies,and clinical applications of HFOs,aiming to provide new theoretical foundation and strategic direction for the precise diagnosis,treatment,and prognostic assessment of epilepsy.
