Clinical efficacy analysis of neuroendoscopic hematoma evacuation via transfrontal and transtemporal approaches for basal ganglia cerebral hemorrhage

Abstract:Objective To analyze the clinical efficacy of neuroendoscopic hematoma evacuation via transfrontal and transtemporal approaches for basal ganglia cerebral hemorrhage.Methods Clinical data of 80 patients with basal ganglia cerebral hemorrhage were analyzed retrospectively.According to the primary anatomical location of the hematoma,patients were assigned to different surgical approaches and divided into a transfrontal approach group(40 cases)and a transtemporal approach group(40 cases).The operative time,intraoperative blood loss,postoperative residual hematoma volume,hematoma evacuation rate,postoperative cerebral edema,postoperative Glasgow Coma Scale(GCS)scores,National Institutes of Health Stroke Scale(NIHSS)scores,Activities of daily living(ADL)scores at 1 month postoperatively,modified Rankin Scale(mRS)scores at 3 months postoperatively,and incidence of complications were compared between the two groups.Results The operative time and postoperative cerebral edema volume in the transfrontal approach group were significantly lower than those in the transtemporal approach group,with statistically significant differences(both P<0.05).There were no statistically significant differences between the two groups in terms of intraoperative blood loss,hematoma evacuation rate,complications,or postoperative rebleeding(all P>0.05).The protection of white matter fiber tracts,improvement in postoperative GCS scores,and NIHSS scores in the transfrontal approach group were significantly better than those in the transtemporal approach group.The ADL scores at 1 month postoperatively and mRS scores at 3 months postoperatively in the transfrontal approach group were also superior to those in the transtemporal approach group,with statistically significant differences(all P<0.05).Conclusions In the treatment of basal ganglia cerebral hemorrhage with neuroendoscopic hematoma evacuation,compared with the transtemporal approach,the transfrontal approach offers advantages such as shorter operative time,relatively milder postoperative cerebral edema,and more significant improvement in prognosis,making it worthy of clinical promotion.
