The evaluative role of serological indicators in short-term functional prognosis of elderly patients with acute cerebral infarction

Abstract:Objective To investigate the evaluative role of visinin-like protein-1(VILIP-1),heat shock protein 47(HSP47),and angiopoietin-like protein 4(ANGPTL4)in the short-term functional prognosis of elderly patients with acute cerebral infarction(ACI).Methods The clinical data of 89 elderly patients with ACI(ACI group)were analyzed retrospectively,all of whom received intravenous thrombolytic therapy.Functional prognosis was assessed using the Activities of Daily Living(ADL)scale 3 months after treatment,and patients were divided into a good functional prognosis group(ADL score>60)and a poor functional prognosis group(ADL score≤60).Additionally,89 healthy individuals undergoing physical examinations during the same period were selected as the control group.The expression levels of serological indicators(VILIP-1,HSP47,ANGPTL4)were compared between the ACI group and the control group.The correlation between these serological indicators and the degree of neurological impairment in elderly ACI patients was analyzed,along with their evaluative efficacy for poor functional prognosis.Results Before thrombolysis,the expression levels of serum VILIP-1,HSP47,and ANGPTL4 in the ACI group were higher than those in the control group(all P<0.05).Before thrombolysis,elderly patients with mild ACI had lower serum levels of VILIP-1,HSP47,and ANGPTL4 compared to those with moderate and severe ACI,while patients with moderate ACI had lower levels than those with severe ACI(all P<0.05).Spearman correlation coefficient analysis revealed that pre-thrombolysis serum VILIP-1,HSP47,and ANGPTL4 were positively correlated with the degree of neurological impairment in elderly ACI patients(all P<0.05).The proportion of patients with large-area infarction and severe neurological impairment was higher in the poor functional prognosis group than in the good functional prognosis group(both P<0.05).Three days after thrombolysis,serum VILIP-1,HSP47,and ANGPTL4 levels decreased in elderly ACI patients with different functional prognoses compared to pre-thrombolysis levels;however,the expression levels of these indicators were higher in the poor functional prognosis group than in the good functional prognosis group both before and 3 days after thrombolysis(all P<0.05).The area under the receiver operating characteristic(ROC)curve(AUC)for the combined evaluation of poor functional prognosis in elderly ACI patients using serological indicators(VILIP-1,HSP47,ANGPTL4)was 0.937(95%CI:0.864-0.978),with a sensitivity of 90.48%and a specificity of 83.58%(P<0.05).Conclusions Serum levels of VILIP-1,HSP47,and ANGPTL4 are significantly elevated in elderly patients with ACI and are correlated with the degree of neurological impairment.Combined detection of these serological indicators holds high evaluation value for the short-term functional prognosis of elderly patients with ACI.
