Analysis of the efficacy and influencing factors of combined extracranial-intracranial vascular bypass surgery for Moyamoya disease

Abstract:Objective To evaluate the efficacy of combined extracranial-intracranial vascular bypass surgery for Moyamoya disease and the influencing factors of postoperative complications.Methods The clinical data of 291 patients(337 sides)with moyamoya disease,who underwent superficial temporal artery to middle cerebral artery(STA-MCA)anastomosis combined with encephalo-duro-arterio-synangiosis(EDAS),were analyzed retrospectively.Postoperative neurological complications were observed.Univariate and multivariate logistic regression analyses were applied to explore the influencing factors.Follow-up at 6 months postoperatively was conducted to determine the occurrence of postoperative cerebral hemorrhage or cerebral infarction.Neurological function was evaluated using the modified Rankin Scale(mRS)at 6 months postoperatively.Results Moyamoya disease surgery was successful in all the 291 patients(337 sides),with neurological complications occurring in 47(16.15%):including 36 of cerebral infarction,8 of epilepsy,and 3 of cerebral hemorrhage.Among them,6 patients experienced severe complications(postoperative cerebral infarction with an increased mRS score postoperatively).There were no perioperative deaths.Multivariate logistic regression analysis indicated that age and preoperative comorbid hypertension were risk factors for severe postoperative complications(both P<0.05).Conclusions Combined extracranial-intracranial vascular bypass surgery is a safe and effective treatment for Moyamoya disease.Postoperative cerebral infarction is a common complication,but only a small proportion of patients exhibit increased mRS scores.
