A case of chordoma combined with pituitary adenoma

Abstract:Objective To investigate the clinical diagnosis and treatment strategies for chordoma combined with pituitary adenoma,aiming to improve the diagnostic and therapeutic proficiency of such rare condition.Methods The clinical data of a 23-year-old male patient was analyzed retrospectively,who presented with weight gain and breast enlargement for 13 years.Preoperative MRI revealed space-occupying lesions in the sellar region and clivus.The tumors were resected via a neuroendoscopic endonasal transsphenoidal approach.Results Gross total resection of the tumor was achieved under microscope.Postoperative pathology confirmed the coexistence of chordoma and prolactin-secreting pituitary adenoma.Immunohistochemistry demonstrated positive staining for synaptophysin(Syn)and prolactin(PRL)in the pituitary adenoma region,while the chordoma region showed positive staining for cytokeratin(CK)and Brachyury gene.Postoperatively,the patient's hormonal levels normalized without new neurological deficits.Conclusions Chordoma combined with pituitary adenoma is rare and prone to misdiagnosis.Comprehensive evaluation incorporating imaging,clinical manifestations,and pathological examination is essential.Neuroendoscopic endonasal transsphenoidal surgery enables total tumor resection,and long-term follow-up is required to monitor recurrence.This case provides valuable insights for the clinical diagnosis and treatment of such complex conditions.
