Relationship between cerebrospinal fluid levels of Parkinson's disease protein 7 and chitinase protein 40 and cognitive impairment in elderly patients with Parkinson's disease

Abstract:Objective To explore the relationship between cerebrospinal fluid(CSF)levels of Parkinson's disease protein 7(PARK7)and chitinase protein 40(YKL-40)and cognitive impairment in elderly patients with Parkinson's disease(PD).Methods A retrospective analysis was conducted on the medical records of 150 elderly patients with PD.Cognitive impairment was assessed using the Montreal Cognitive Assessment(MoCA)scale,with patients scoring<26 classified into the mild cognitive impairment(MCI)group(n=64)and those scoring≥26 into the non-MCI group(n=86).CSF levels of PARK7 and YKL-40 were compared between the two groups.Risk factors influencing the development of cognitive impairment in elderly PD patients were analyzed,along with the predictive value of these biomarkers for cognitive impairment.Results The proportion of patients with Hoehn-Yahr(H-Y)stagesⅢ~Ⅳand CSF levels of PARK7 and YKL-40 were significantly higher in the MCI group than in the non-MCI group(all P<0.05).Multivariate regression analysis revealed that H-Y stage,CSF PARK7 level,and CSF YKL-40 level were independent risk factors for cognitive impairment in elderly PD patients(all P<0.05).Receiver operating characteristic(ROC)analysis indicated that the optimal cut-off values for predicting cognitive impairment in elderly PD patients were 33.89 μg/L for CSF PARK7 and 3.63 mg/L for CSF YKL-40.The combined sensitivity,specificity,and area under the curve(AUC)were 82.81%,98.84%,and 0.951(95%CI:0.903-0.980),respectively.Conclusion CSF levels of PARK7 and YKL-40 are abnormally elevated in elderly PD patients with cognitive impairment.Clinical detection of CSF PARK7 and YKL-40 levels can serve as indicators for predicting cognitive impairment in elderly PD patients,with higher predictive value when used in combination.
