Endoscopic resection of an intracavernous angioleiomyoma:a case report and literature review

Abstract:Objective To summarize the surgical experience in the resection of an intracavernous sinus angioleiomyoma.Methods The case data of one patient with an intracavernous sinus angioleiomyoma was analyzed retrospectively.The tumor was resected via a neuroendoscopic endonasal transsphenoidal approach.Results The tumor was completely resected.During the 5-month follow-up after surgery,no evidence of tumor recurrence on MRI.The symptoms of headache and ocular swelling/pain disappeared,and diplopia gradually improved.Conclusions Endoscopic transnasal surgery can achieve favorable outcomes for patients.Intracavernous sinus angioleiomyomas may exhibit a"flame sign"on contrast-enhanced MRI,and is prone to clinical misdiagnosis,and definitive diagnosis relies on pathological examination.Pathologically,these tumors are characterized by smooth muscle surrounding vascular structures,with positive immunohistochemical staining for smooth-muscle actin(SMA)and CD34,and negative staining for epithelial membrane antigen(EMA)and glial fibrillary acidic protein(GFAP).
