Clinicopathological analysis of spinal Rosai-Dorfman disease

Abstract:Objective To investigate the clinicopathological characteristics,diagnosis,and differential diagnosis of spinal Rosai-Dorfman disease(RDD).Methods The clinicopathological data of 3 patients with spinal RDD were analyzed retrospectively,including histological characteristics,immunohistochemistry,and molecular pathology,along with a review of relevant literature.The patients comprised 1 male and 2 females.Results Laboratory examinations revealed increased levels of monocytes,lymphocytes,reticulocytes,and platelet hematocrit,with decreased levels of albumin,total protein,and cystatin C.Imaging studies(CT and MRI)demonstrated space-occupying lesions.Microscopic examination revealed nodular aggregates of histiocytes mixed with lymphocytes and plasma cells were observed on a background of fibrosis or collagenization.Typical"emperipolesis"was observed in histiocytes,characterized by the presence of engulfed lymphocytes,plasma cells,and erythrocytes within the cytoplasm.Immunophenotypically,histiocytes were positive for S-100 and CD68,and negative for CD1a.Lymphoma gene rearrangement testing was negative.Conclusions Spinal RDD is a rare benign histiocytic proliferative disorder,lacking characteristic clinical manifestations,serological,or imaging findings.Diagnosis relies on pathological morphology and immunohistochemistry.
