Effect of catheterization direction on postoperative complications after burr-hole drainage for chronic subdural hematoma

Abstract:Objective To investigate the impact of catheter placement direction on postoperative complications after burr hole drainage for chronic subdural hematoma(CSH).Methods Clinical data of 375 patients with CSH were analyzed retrospectively,who underwent burr hole drainage,involving 417 sides of CSH.Based on the location of the drainage tube tip,patients were categorized into three groups:anterior placement group(263 cases,287 sides),middle placement group(83 cases,95 sides),and posterior placement group(29 cases,35 sides).Results In the anterior placement group,17 sides experienced postoperative recurrence,1 side developed postoperative intracranial infection,2 sides sustained brain injury,and 4 sides suffered cerebrospinal fluid(CSF)overdrainage.In the middle placement group,8 sides had recurrence,with no cases of postoperative intracranial infection,brain injury,or CSF overdrainage.In the posterior placement group,2 sides experienced recurrence,2 sides sustained brain injury,with no intracranial infection or CSF overdrainage.No statistically significant differences were found in recurrence rates between the three groups(χ2=0.772,P=0.680).Similarly,there were no significant differences in postoperative intracranial infection or CSF overdrainage(χ2=0.454,P=0.797;χ2=1.829,P=0.401).The incidence of brain injury complications in the middle placement group was lower than that in the other two groups,and the difference was statistically significant(χ2=8.869,P=0.012).Conclusions The direction of catheter placement has no significant impact on the recurrence rate,postoperative intracranial infection,and CSF overdrainage after burr hole drainage for CSH.However,it does have a significant impact on postoperative brain injury.Individualized treatment plans should be adopted in clinical practice.
