Clinical application of balloon-assisted minimally invasive approach for deep brain lesions

Abstract:Objective To evaluate the efficacy and safety of balloon-assisted minimally invasive approach combined with diffusion tensor imaging(DTI)navigation for deep brain lesions.Methods Clinical data of 40 patients with deep brain lesions(≥2.5 cm from the cortical surface)were analyzed retrospectively.These patients were divided into two groups based on the use of a neuro-balloon:the balloon group(n=18,using neuronavigation combined with a minimally invasive balloon-assisted channel)and the conventional group(n=22,using neuronavigation combined with a traditional brain retractor).The operative time,extent of resection,complication rates,and Karnofsky Performance Scale(KPS)scores were compared.Multivariate logistic regression analysis was performed to identify prognostic factors.Results The balloon group had a significantly shorter mean operative time compared to the conventional group[(248.6±64.4)min vs.(328.4±82.6)min,P=0.002],a higher rate of total resection(94.44%vs.68.18%,P=0.001),and better postoperative quality of life(83.33%vs.54.55%,P=0.028).Multivariate logistic regression analysis revealed that the use of the minimally invasive balloon approach(OR=0.32,95%CI:0.15-0.68,P=0.004)and DTI technology(OR=0.45,95%CI:0.22-0.92,P=0.029),and total resection(OR=0.38,95%CI:0.18-0.79,P=0.010)were protective factors for favorable postoperative quality of life.Conclusions For deep brain lesions,the minimally invasive balloon approach combined with DTI technology significantly enhances surgical safety and efficacy,thus improving postoperative quality of life for patients.
