Advances in the Application of Neurophysiological Monitoring in Facial Spasm
Abstract:Primary facial spasm is caused by vascular compression at the root exit zone (REZ) of the facial nerve, leading to increased excitability of the facial nerve and muscle twitching. Microvascular decompression is the most effective treatment for facial spasm, but the complex anatomy of the cerebellopontine angle may lead surgeons to misidentify the responsible vessel, resulting in incomplete decompression of the facial nerve. Intraoperative neurophysiological monitoring can effectively prevent neural structure damage, correctly identify the responsible vessel, and improve the success rate of surgery. Commonly used neurophysiological monitoring parameters include free electromyography, brainstem auditory evoked potentials, facial motor evoked potentials, abnormal muscle responses, Z-L response, and blink reflex. This article reviews the recent research progress on these indicators.
