Application of quantitative electroencephalogram indicators on the affected side in the diagnosis of hippocampal sclerosis in unilateral temporal lobe epilepsy

Abstract:Objective To investigate the application of quantitative electroencephalogram(QEEG)indicators on the affected side in the diagnosis of hippocampal sclerosis(HS)in patients with unilateral temporal lobe epilepsy(TLE).Methods Clinical data of 107 patients with TLE were analyzed retrospectively,who were divided into left temporal lobe epilepsy group(LTLE group,51 cases)and right temporal lobe epilepsy group(RTLE group,56 cases)according to the affected side.Additionally,53 healthy individuals undergoing routine physical examinations during the same period were enrolled as the normal control group(NC group).Propensity score matching was applied,resulting in 50 matched groups for comparison(50 cases each in the LTLE,RTLE,and NC groups).The QEEG indicators(including α,β,θ,δ frequency bands,and relative power ratios)and hippocampal metabolite ratios,including N-acetylaspartate(NAA)/[choline(Cho)+creatine(Cr)],NAA/Cr,and NAA/Cho,were compared between the groups.Multivariate logistic regression analysis was performed to explore independent risk factors for hippocampal sclerosis in unilateral TLE.The correlation between QEEG indicators and hippocampal metabolite ratios was analyzed.The area under curve-receiver operating characteristic(AUC-ROC)was used to evaluate the value of QEEG on the affected side in the diagnosis of hippocampal sclerosis in patients with unilateral TLE.Results The α and β frequency bands of QEEG in the LTLE and RTLE groups were significantly lower than those in the NC group,while the θ and δ frequency bands and the(δ+θ)/(α+β)ratio(DTABR)were significantly higher.The NAA/(Cho+Cr),NAA/Cr,and NAA/Cho ratios in the LTLE and RTLE groups were significantly lower than those in the NC group.Correlation analysis revealed that NAA/Cr,NAA/Cho,and NAA/(Cho+Cr)were independently correlated with the α,β,θ,δ frequency bands and DTABR in QEEG indicators.Multivariate logistic regression analysis indicated that the α,β,θ,δ frequency bands,DTABR,NAA/(Cho+Cr),NAA/Cr,and NAA/Cho were all independent risk factors for hippocampal sclerosis.The AUC-ROC for the combined diagnosis of hippocampal sclerosis using the five QEEG indicators(α,β,θ,δ frequency bands,and DTABR)was 0.798(95%CI:0.732-0.914),significantly higher than the diagnostic efficacy of either indicator alone.Conclusion QEEG can quantitatively analyze the abnormal brain discharge in patients with unilateral TLE,thus having a good diagnostic value for hippocampal sclerosis.
