Comparative study of ulnar nerve in situ decompression and transposition in the treatment of different clinical types of cubital tunnel syndrome

Abstract:Objective To investigate the effect of in situ decompression and transposition preposition in the treatment of different clinical types of cubital tunnel syndrome(CuTS).Methods The clinical data of 224 patients with CuTS were analyzed retrospectively,including 36 mild,128 moderate and 60 severe patients.In the mild group,18 patients underwent in situ decompression and 18 underwent anterior transposition.In the moderate group,64 patients were treated with in situ decompression and 64 with anterior transposition.In the severe group,30 patients received in situ decompression and 30 underwent anterior transposition.The ulnar nerve function evaluation and nerve electrophysiological indexes were used to compare and analyze the postoperative efficacy.Results In the mild group,the good or excellent clinical outcome rate was 83.3%for in situ decompression and 88.9%for anterior transposition.In the moderate group,the rates were 87.5%and 85.9%,respectively.In the severe group,the rates were 70%for in situ decompression and 90%for anterior transposition.No statistically significant differences were observed between the two surgical approaches in the mild group and moderate group(all P>0.05).In severe group,there was a statistically significant difference between the two surgical methods(P<0.05).In the mild group and moderate group,significant improvements in sensory nerve conduction velocity(SCV)and motor nerve conduction velocity(MCV)were observed postoperatively compared with preoperative values,but no statistically significant differences were found between the two surgical methods(all P>0.05).In the severe group,postoperative SCV and MCV also improved significantly compared with preoperative values,with anterior transposition showing superior results to in situ decompression(P<0.05).Conclusions For mild to moderate CuTS,the two surgical methods have comparable effects.However,for severe CuTS,the effect of anterior transposition is better than that of in situ decompression.The specific surgical method should be determined according to the preoperative classification of patients.
