Diagnostic value of combined three-dimensional dual-echo steady state and three-dimensional short time inversion recovery spin echo imaging sequences in magnetic resonance neuroimaging for brachial plexus injury

Abstract:Objective To explore the application value of magnetic resonance neuroimaging(MRN)using the three-dimensional dual-echo steady state(3D-DESS)sequence combined with the three-dimensional short time inversion recovery spin echo imaging(3D-STIR-SPACE)sequence in the diagnosis of brachial plexus injury(BPI).Methods The clinical data of 59 patients with suspected BPI were analyzed retrospectively,all of whom underwent 3D-DESS sequence and 3D-STIR-SPACE sequence scanning.The MRN image characteristics of BPI were analyzed.The display of brachial plexus,brachial plexus background suppression score,and contrast to noise ratio(CNR)on different images were compared.The diagnostic performance of 3D-DESS sequence,3D-STIR-SPACE sequence,and their combined application in BPI was also compared.Results The 3D-DESS sequence scanning showed slightly higher intensity in injured nerves,and enhanced scan showed isointensity.The 3D-STIR-SPACE sequence scanning showed high intensity in injured nerves,and enhanced scan showed increased intensity in both injured nerves and surrounding soft tissues.The nerve display score,background suppression score and CNR of the brachial plexus were higher in the 3D-STIR-SPACE sequence than in the 3D-DESS sequence(P<0.05).The sensitivity and accuracy of different sequences and their combination for diagnosing BPI were significantly different(P<0.05).The diagnostic sensitivity and accuracy of combined application of 3D-DESS sequence and 3D-STIR-SPACE sequence were higher than those of 3D-DESS sequence(P<0.05).Conclusions MRN is an important imaging method for clinical diagnosis of BPI.3D-STIR-SPACE sequence can better suppress surrounding background signals and clearly display the brachial plexus.The combination of 3D-DESS sequence and 3D-STIR-SPACE sequence scanning can improve the sensitivity and accuracy in diagnosing BPI.
