Detection value of serum biomarkers in epileptic patients with different degrees of electroencephalogram abnormalities

Abstract:Objective To analyze the clinical significance of serum levels of Toll-like receptor 4(TLR4),Caspase-3,and brain-derived neurotrophic factor(BDNF)in epileptic patients with different degrees of electroencephalogram(EEG)abnormalities.Methods The clinical data of 140 epileptic patients(observation group)were analyzed retrospectively,and 140 healthy individuals undergoing physical examinations during the same period served as the control group.According to the degree of EEG abnormality,patients in the observation group were divided into mild abnormality group,moderate abnormality group and severe abnormality group.The differences in serum TLR4,caspase-3,and BDNF levels between the observation and control groups and between the patients with different degrees of EEG abnormalities,were compared.The relationship between these levels and the severity of EEG abnormalities was analyzed.The receiver operating characteristic(ROC)curves were used to evaluate the values of serum TLR4,caspase-3,and BDNF levels in differentiating epileptic patients with different degrees of EEG abnormalities.Results Compared with the control group,serum TLR4 and caspase-3 levels were higher,and BDNF level was lower in the observation group(P<0.05).EEG examination revealed that there were 73 cases of mild abnormalities,40 of moderate abnormalities,and 27 of severe abnormalities.The serum TLR4 and caspase-3 levels in the mild abnormality group,the moderate abnormality group,and the severe abnormality group increased in order,while serum BDNF levels in the three groups decreased in order(P<0.05).Spearman correlation analysis showed that serum TLR4 and caspase-3 levels were positively correlated with the severity of EEG abnormalities in epileptic patients(r=0.699,0.647,both P<0.01),while BDNF levels were negatively correlated(r=-0.632,P<0.01).The area under the ROC curve(AUC)values of serum TLR4,caspase-3,and BDNF for diagnosing mild,moderate,and severe EEG abnormalities in epilepsy were all greater than 0.7(P<0.01).Conclusions The serum TLR4 and caspase-3 levels increase,and BDNF level decreases with the severity of EEG abnormalities.Detecting the three serum biomarkers can help to evaluate the degree of EEG abnormalities in patients and provide reference for clinical treatment.
