The predictive value of combined serum biomarkers for cognitive dysfunction in patients with severe traumatic brain injury

Abstract:Objective To analyze the predictive value of combined serological indicators for cognitive dysfunction in patients with severe traumatic brain injury(sTBI).Methods A total of 118 patients with sTBI were enrolled and divided into a dysfunction group and a non-dysfunction group based on the occurrence of cognitive dysfunction.Univariate and multivariate logistic analyses were conducted to identify risk factors for cognitive dysfunction in sTBI.Receiver operating characteristic(ROC)curves were plotted to analyze the diagnostic value of combined serological indicators.Results Compared with the non-dysfunction group,the dysfunction group had a higher proportion of patients aged older and with an education level of middle school or below,as well as elevated levels of calcitonin(CT),hypersensitive C-reactive protein(hs-CRP),serum amyloid A(SAA),interleukin-6(IL-6),and interleukin-8(IL-8).Additionally,the Glasgow coma scale(GCS)scores on admission were lower in the dysfunction group(all P<0.05).Logistic analysis revealed that older age,an education level of middle school or below,lower GCS scores upon admission,and elevated levels of serum CT,hs-CRP,SAA,IL-6,and IL-8 were risk factors for cognitive dysfunction in sTBI(all P<0.05).ROC curve analysis showed that the areas under the curve(AUC)for serum CT,hs-CRP,SAA,IL-6,IL-8,and the combined diagnosis of cognitive dysfunction in sTBI were 0.705,0.702,0.862,0.793,0.799,and 0.957,respectively.The combined detection of the five indicators was significantly more accurate than individual detections(P<0.05).Conclusions Clinically,the occurrence of cognitive dysfunction can be predicted by detecting the serum levels of CT,hs-CRP,SAA,IL-6 and IL-8 in patients with sTBI,so as to intervene early and improve the prognosis of patients.
