Endoscopic third ventriculostomy combined with endoscopic tumor biopsy for the treatment of hydrocephalus caused by third ventricular tumors:a retrospective analysis of 33 cases

Abstract:Objective To investigate the efficacy and safety of endoscopic third ventriculostomy(ETV)combined with endoscopic tumor biopsy(ETB).Methods The clinical data of 33 patients with space-occupying lesions in the third ventricle(including the pineal and thalamic regions)complicated by non-communicating hydrocephalus were analyzed retrospectively,with a median age of 23 years.All patients underwent ETV combined with ETB and received single-burr hole except 2 cases with two burr hole and two trajectories in the same operation respectively.Results The histological diagnoses included 13 germinomas,3 nongerminal matrix germ cell tumours,4 gliomas,7 pineal tumors,3 benign cysts,and 1 case metastatic tumor,1 case atypical teratoid rhabdoid tumor and 1 case meningioma(based on imaging diagnosis).The rate of conclusive histological diagnosis was 91%(30/33).One patient developed postoperative intraventricular hemorrhage without clinical sequelae,1 patient of epidural hematoma and 3 patients required ventriculoperitoneal shunt surgery postoperatively.Conclusions ETV combined with ETB is safe and effective.The importance of precise preoperative trajectory and entry point planning is emphasized to optimize surgical outcomes and reduce risks.
