A study on the prevalence of temporomandibular disorder in patients with traumatic brain injury one year after discharge

Abstract:Objective To evaluate the prevalence of temporomandibular disorders(TMD)in patients with traumatic brain injury(TBI)1 year after discharge.Methods A total of 87 TBI patients were selected as the case group,and 174 patients with general trauma during the same period of time were selected as the control group according to the principle of the same gender and the age difference of not more than 2 years in a 1∶2 case-control study.The incidence of TMD was compared between the two groups to explore the correlation between TBI symptoms and the onset of TMD.Results The incidence of TMD was 56.32%in the case group(87 TBI patients)and 8.05%in the control group,with a statistically significant difference between the two groups(P<0.01).Among the 87 TBI patients,46 presented headaches with a TMD incidence of 67.39%,and 41 had no headaches with a TMD incidence of 43.90%.The difference in TMD incidence between patients with and without headaches was statistically significant(P=0.027).Among the clinical symptoms of TBI patients,headache was the only symptom correlated with TMD(r=0.515,P<0.01).Logistic regression analysis showed that headache,TBI,female gender,and depression were risk factors for the onset of TMD(P<0.05,OR>1),while older age and obesity were associated with a lower risk of TMD(P<0.05,OR<1).Conclusions Patients with TBI are more prone to problems with the temporomandibular joint and have a higher incidence of TMD.The accompanying symptom of headache in TBI patients is correlated with the onset of TMD.
