Relationship between baseline characteristics and prognosis in patients with spontaneous subarachnoid hemorrhage

Abstract:Objective To explore the relationship between baseline characteristics and prognosis in patients with spontaneous subarachnoid hemorrhage(SAH).Methods The data of 80 patients with spontaneous SAH were analyzed retrospectively and divided into a good prognosis group(GOS 4-5 points,n=49)and a poor prognosis group(GOS≤3 points,n=31)according to the Glasgow Outcome Scale(GOS).Comparing the differences of baseline characteristic indexes between the two groups,the relationship between baseline characteristics and prognosis was analyzed using multifactorial analysis,and a nomogram model was established and validated.Results Compared with the good prognosis group,patients in the poor prognosis group had higher age,higher World Federation of Neurological Surgeons(WFNS)scale,Hunt-Hess grades,and modified Fisher grades.They also had a higher incidence of acute hydrocephalus,delayed cerebral ischemia,lung infection and intracranial hemorrhage,larger aneurysm diameters,aneurysms more frequently located in the middle cerebral artery,shorter time from onset to surgery,more frequent use of craniotomy clipping and stent-assisted embolization,and more frequent use of extraventricular drainage for cerebrospinal fluid(all P<0.05).The multivariate logistic regression model revealed that WFNS scale,delayed cerebral ischemia,pulmonary infection,and intracranial hemorrhage were influential factors predicting the prognosis of spontaneous SAH(all P<0.05).The area under the curve(AUC)for predicting the prognosis of spontaneous SAH using the nomogram in the modeling group was 0.945.Conclusions The baseline characteristics may be associated with the prognosis of patients with spontaneous SAH.Higher WFNS grades and the presence of delayed cerebral ischemia,pulmonary infection,and intracranial hemorrhage are indicative of a poorer prognosis in patients with spontaneous SAH.
