Impact of sphenoid sinus anatomy on the vertical growth of nonfunctional pituitary macroadenomas

Abstract:Objective To investigate the influence of sphenoid sinus related anatomical structure on the vertical expansion direction of nonfunctioning pituitary macroadenoma.Methods The imaging data of 65 patients with nonfunctioning pituitary macroadenoma were analyzed retrospectively.According to the suprasellar expansion value(SEV),the patients were divided into suprasellar group(SEV>0)and infrasellar group(SEV≤0).The types of the sphenoid sinus,the deviation of the main septum,the volume of the sphenoid sinus and the maximum width of main septum of the sphenoid sinus were compared between the two groups.Multivariate logistic regression analysis was performed to identify significant influencing factors.Results Compared with the infrasellar group,the suprasellar group had better sphenoid sinus gasification(P=0.016),wider main septum of the sphenoid sinus(P=0.003),more middle main septum of the sphenoid sinus(P=0.048)and larger volume of the sphenoid sinus(P=0.040)with statistically significant differences between the two groups.Multivariate logistic regression analysis showed that the maximum width of main septum of the sphenoid sinus(OR=6.23,P=0.013)and the volume of the sphenoid sinus(OR=0.72,P=0.024)were independent predictors of vertical expansion in nonfunctional pituitary macroadenomas.Conclusions The smaller the size of the sphenoid sinus and the wider the main septum of the sphenoid sinus are associated with a higher tendency for nonfunctional pituitary macroadenomas to grow suprasellarly.
