Pathogenic bacteria and risk factors for intracranial infections after hematoma removal for cerebral hemorrhage

Abstract:Objective To analyze the characteristics of pathogenic bacteria and risk factors of intracranial infection after hematoma removal for cerebral hemorrhage to provide reference for clinicians to prevent postoperative intracranial infection and reduce the infection rate.Methods The clinical data of 422 patients,who underwent hematoma removal surgery for cerebral hemorrhage were analyzed retrospectively.The patients were divided into infection group and control group based on whether intracranial infection occurred.The propensity score matching(PSM)method was used to match patients in the two groups at a ratio of 1:1.The distribution characteristics of pathogenic bacteria in the infection group were analyzed,and multivariate logistic regression analysis was used to observe the risk factors of intracranial infection after hematoma removal and establish relevant regression equations.Results After PSM matching,132 pairs(a total of 264 patients)were successfully matched and included in the study.Among the 132 patients in the infection group,32 strains of pathogens were isolated,including 15 strains of Gram-positive bacteria,16 strains of Gram-negative bacteria,and 1 strain of fungi.Multivariate analysis results showed that ASA score>3,long hospital stay,long duration of drainage tube placement,low preoperative albumin level,and high body temperature on the third day after surgery were independent risk factors for intracranial infection after hematoma removal for cerebral hemorrhage.A combined diagnostic model was obtained by linearly fitting the above risk factors:logit(P)=-57.166+1.05×ASA score>3+0.104×hospital stay duration+0.169×drainage tube placement duration-0.114×preoperative albumin level+1.512×high body temperature on the third day after surgery.The area under the curve(AUC)was 0.848,with the diagnostic sensitivity was 87.10%and specificity was 65.20%.Conclusions ASA score>3,long hospital stay,long duration of drainage tube placement,low preoperative albumin level,and high body temperature on the third day after surgery are independent risk factors for intracranial infection after hematoma removal for cerebral hemorrhage.The combined diagnostic model established based on the above indicators has high diagnostic value for intracranial infection.
