Analysis of the factors influencing aneurysm rupture and stroke after treatment in Moyamoya disease complicated with intracranial aneurysm

Abstract:Objective To investigate the factors influencing aneurysm rupture and stroke after treatment in moyamoya disease(MMD)complicated with intracranial aneurysm(IA).Methods The clinical data of 245 patients with MMD combined with IA were analyzed retrospectively,which were divided into rupture group(n=71)and non-rupture group(n=174)based on whether IA rupture occurred.The univariate and logistic regression analyses were used to identify risk factors for IA rupture.Patient follow-up data were recorded and the COX regression analysis was used to identify risk factors for post-treatment stroke.Results The rupture rate of IA in MMD patients was 28.98%.Age>52.5 years old(OR=1.997)and systolic blood pressure(SBP)>160mmHg(OR=2.042)were independent risk factor for the rupture of IA in MMD,while the internal carotid artery(OR=0.353)was the opposite.The incidence of stroke was 6.53% during the follow-up period.Medium and large aneurysm(HR=3.940)and aneurysm rupture history(HR=3.900)were independent risk factors for stroke in MMD combined with IA.Conclusions Patients with MMD combined with IA who are older and have higher SBP have a higher risk of rupture.Patients with medium and large IAs and a history of IA rupture have a higher risk of stroke.
