Correlation between hemorrhagic transformation after intravenous thrombolysis and white matter hyperintensity in patients with acute ischemic stroke

Abstract:Objective To investigate the correlation between hemorrhagic transformation and white matter hyperintensity(WMH)in patients with acute ischemic stroke(AIS)after intravenous thrombolysis.Methods The clinical data of 137 patients with AIS were analyzed retrospectively,all of whom underwent intravenous thrombolysis therapy,and were divided into hemorrhagic transformation group(53 cases)and non-hemorrhagic transformation group(84 cases)according to whether hemorrhagic transformation occurred.Univariate and logistic regression analysis were performed to analyze the risk factors of hemorrhagic transformation in AIS patients after treatment.Results There were significant differences between the two groups in age,NIHSS score,hypertension,onset to treatment time,and WMH degree(P<0.05).Logistic analysis showed that moderate to severe WMH(OR=2.796,95%CI:1.450-5.391)was a risk factor for hemorrhagic transformation in patients with AIS after treatment(P<0.05).Conclusions WMH is a risk factor for hemorrhagic transformation after thrombolytic therapy in patients with AIS,and patients with moderate to severe WMH have a higher risk of hemorrhagic transformation.
