Superselective intra-arterial thrombolysis combined with embolectomy by stents for acute middle cerebral artery occlusion

Abstract:Objective To investigate the safety and efficacy of superselective intra-arterial thrombolysis combined with embolectomy by stents for acute middle cerebral artery (MCA) occlusion. Methods The clinical data of 20 patients with acute MCA occlusion and accessing to emergency channel were analyzed retrospectively. The initial diagnosis was confirmed by CTA or MRA in all the cases. After DSA assessment, the micro-catheter was superselected to the occlusion site and urokinase was used for thrombolysis. But the blood vessel was not opened, a Solitaire AB stent was performed after heparinization. The National Institute of Health stroke scale (NIHSS) scores before, 1, 7, 14 days after operation and at discharge were compared. Results MCA occlusion achieved complete recanalization in 10 cases, partial recanalization in 8 and hemorrhage occurred in 3 after operation. The clinical symptoms were improved in 18 patients and 2 patients died. The NIHSS scores before operation and at discharge were 10.79±5.84 and 7.26±5.61 respectively, with a statistically significant difference (P < 0.01). Conclusion The treatment of superselective intra-arterial thrombolysis combined with embolectomy by stents for acute MCA occlusion can significantly improve recanalization rate and prognosis, showing good clinical efficacy and safety.
