Surgical treatments for anterior circulation aneurysms via modified supra-orbital approach combined with external ventricular drainage

Abstract:Objective To investigate the advantages and efficacy of the surgical treatments for anterior circulation aneurysms via modified supra-orbital approach combined with external ventricular drainage (EVD) . Methods Clinical data of 89 anterior circulation aneurysms in 87 patients were analyzed retrospectively. The mini bone windows were adjusted according to the locations of the aneurysms. The aneurysms were clipped via the modified supra-orbital approach and EVD was performed. Seventy anterior-posterior communicating artery aneurysms were observed in 68 patients and 19 middle cerebral artery aneurysms in 19 patients. The aneurysms re-ruptured during the surgery in 2 patients, who received decompressive craniectomy after the clipping. Results All the aneurysms were successfully clipped in 87 patients. The postoperative CTA showed no residual aneurysms in all the patients. Eighty-five patients were followed up for 6 to 12 months, and GOS results showed 5 points in 66 patients, 4 points in 16 and 3 points in 3. Two patients were lost to follow-up. Conclusion The application of modified supra-orbital approach combined with EVD is proved to be a safe, feasible, effective, minimal invasive and practical way to treat anterior circulation aneurysms, thus deserving to be popularized.
