Meta-analysis of efficacy and complications after direct bypass vs indirect bypass for adult moyamoya disease

Abstract:Objective To make a systematic evaluation of the occurrence of stroke, perioperative complication and revascularization of direct bypass and indirect bypass for symptomatic adult moyamoya disease by Meta-analysis. Methods The related literature about direct bypass surgery including direct and combined bypass and indirect bypass surgery for symptomatic adult moyamoya disease was searched in Pub Med, Web of Science, Cochrane, Google scholar, CNKI and Wanfang database according to the specific inclusion and exclusion criteria. The quality of selected literature was evaluated by Newcastle-Ottawa. The differences in stroke, perioperative complications and revascularization between the two groups were analyzed and compared by Meta-analysis using random-effect model or fixed-effect model. Results A total of 11 literatures were analyzed in the Meta-analysis. There were 414 hemispheres in direct bypass group and 542 in indirect bypass group. Compared with indirect bypass group, the rates of stroke and bad angiographic revascularization were lower in direct bypass group (P < 0.05) . There was no significant difference in perioperative complication rates between the 2 groups (P> 0.05) . Conclusions The direct and combined bypass surgery is superior over the indirect bypass surgery in prevention of future stroke and revascularization in adult moyamoya patients with symptoms.
