Efficacy evaluation of cross positioning with two points and three lines in microinvasive craniopuncture for hypertensive cerebellar hematoma

Abstract:Objective To discuss the application of cross positioning with two points and three lines in microinvasive craniopuncture for cerebellar hematoma. Method Clinical data of 35 patients with hypertensive cerebellar hematoma were analyzed retrospectively, and cross positioning with two points and three lines was performed in all the patients before the operation, then, microinvasive craniopuncture was implemented. The positioning accuracy rate, therapeutic effect and complications were analyzed. Result The puncture needles reached the hematoma by one-time puncture, and the precision rate was 100%in 35 patients. CT 6 h after the operation showed the deviations of the puncture point and puncture direction between preoperation and postoperation was less than or equal to 0.5 cm in 22 patients (62.9%), and the deviations from more than 0.5 cm to 1 cm in 13 (37.1%). Compared with preoperation, the NIHSS score obviously decreased and the Barthel index significantly increased after the operation, and the differences were statistically significant (P<0.05). The postoperative complications included pulmonary infection in 3 patients, digestive tract hemorrhage in 2, urinary tract infection in 2 and heart failure in 1 patient. The patients were cured in 26 (74.3%), improved in 6 (17.1%), achieved bad outcome in 2 (5.7%), and died in 1 (2.9%). The total response rate was achieved 91.4%. Conclusions The cross positioning with two-points and three-lines before the surgery is useful for locating the hematoma with a high accuracy rate, mini-invasion, simple operation, fast hematoma removal and less complications. In addition, it can improve the clinical effects and prognosis, has high clinical application value and is worthy to be popularized for hypertensive cerebellar hematoma.
