Surgical strategy for giant pituitary adenoma with suprasellar growth

Abstract:Objective To explore the clinical value of staged surgery for giant pituitary adenoma with suprasellar growth.Methods The clinical data of 10 patients suffering from giant pituitary adenoma with suprasellar growth were analyzed retrospectively.All patients received the primary surgery by endoscopic transsphenoidal approach resection of the tumor.According to the position of postoperative residual tumor,a suitable staged surgical approach was chosen.Results In primary surgery,tumor was totally resected in 2 cases,subtotally resected in 3 and partially resected in 5 with staged surgery.The residual tumor descended into the sella turcica in 2 patients who received second operation by endoscopic transsphenoid approach,residual tumor located in the suprasellar region in 3 who received second minimally invasive surgery by eyebrow supraorbital keyhole approach.In 5 patients with staged operation,total resection was done in 4 cases and subtotal resection in 1.There were no cerebrospinal fluid leakage,intracranial infection and other serious complications after the operation.The follow-up was performed 3 to 60 months,with an average of 26 months.The pituitary function returned to normal after hormone replacement therapy for 3 to 6 months.Conclusions A suitable staged surgery assisted with gamma knife,medicine and other comprehensive treatment.could be an effective method for giant pituitary tumor with suprasellar growth.
